Registration
Starting in Fall 2024, facilities using AHS Identity & Access Management (IAM) will transition away from the IDSM User Registration Form. Instead, IDSM Authorized Approvers will manage IDSM access directly through the AHS IAM application.

For IDSM user registration, please contact the IDSM Authorized Approver at your facility. If you are unsure who your IDSM Authorized Approver is, please reach out to the eHealth Services Provider Support at

IDSM Authorized Approver Registration
• Download and save the form to your computer.
• If required, download Adobe Reader.
• Ensure all required information is entered accurately before printing the form for authorization.
• Authorized custodian signature is required.
Submitting the Form
- Option 1: Email eHealthFormsManagement@gov.ab.ca and request a submission link. (Go-Anywhere File Transfer email). Do NOT email completed forms directly.
- Option 2: Fax completed form to eHealth Services: 1-844-630-0877
IDSM User Management
For IDSM Authorized Approvers, please review the material below to learn how to login to AHS IAM and request IDSM access for users at your facility.
Exception facilities: Non-Alberta Netcare facilities, such as Occupational Health and Safety [OHS], correctional facilities, long term care or continuing care facilities, police services and auxiliary hospitals, can continue to use the same
IDSM Registration Form.
