Immunization Direct Submission Mechanism (IDSM)
Access & User Registration
The Immunization Direct Submission Mechanism (IDSM) is a standalone web application that facilitates the electronic submission of the immunization and assessment event information from health practitioners to the provincial immunization repository (Imm/ARI) through a web browser.
A user must complete an IDSM User Registration form and be authorized by a health practitioner to obtain access to IDSM.

Need access to IDSM?
- Click on the link above to save the IDSM form to your computer and open it from where you saved it (e.g. desktop or the download folder).
- Adobe Reader is required to open the form, click here to download Adobe Reader.
IMPORTANT:
- Each user needs their own individual email address when registering for IDSM (no generic group e-mails).
- Type forms to avoid rejections due to illegibility and fields not displaying within the smart form
Submitting your IDSM User Registration Form

- Fill out the registration form.
- Send a message to eHealthFormsManagement@gov.ab.ca with Subject Line: IDSM User Access Request - <Type your site name here>
- DO NOT attach form to this email.
- You will receive an email response with a link to securely upload your form(s).
