Best Possible Medication History (BPMH) Form – Medication Reconciliation

Medication Reconciliation (MedRec) aims to standardize and support safe medication practices across Alberta Health Services. Through MedRec, an accurate list of all of a patient’s prescribed medications, non-prescription and over-the-counter drugs, supplements and herbal remedies is collected and communicated to other healthcare professionals as they become involved in the care of the patient.  This record is compared with new medication orders and helps to provide better, safer care for patients.

The Best Possible Medication History (BPMH) form is pre-populated with dispensed prescriptions recorded in the Pharmaceutical Information Network (PIN) and can be used to complete the BPMH, admission orders and retained as a permanent part of the chart.

The Information Sheet below will tell you more about the BPMH form and how it supports the MedRec process. The BPMH Form Instructions Sheet will tell you how to fill out the form.  The BPMH Form Frequently Asked Questions (FAQ) Sheet answers some commonly asked questions about the BPMH Form.
